What is the PCN Mental Health Service?
We are a team of mental health wellbeing coaches and a mental health practitioner providing a dedicated wellbeing support service which is focused on practical mental health management. We use a collaborative approach to help you develop your own tools and coping strategies. This can help you build on your existing strengths and resources to support your mental health.
Our mental health practitioner can also support assessment and formulation of new diagnosis and review existing mental health conditions where the risk is low.
What the service does:
- The coaches can work with you to develop tools and techniques to enable you to use your own resources to generate actionable solutions to your mental health problems.
- These tools can help you with low mood, anxiety management, stress management and can include behavioural activation and coping skills.
- We work together to set personalised goals.
- The mental health practitioner is able to liaise closely with the local community mental health team to get specialist support when needed. They can also prescribe medication if needed for depression and anxiety.
- Appointments can be face to face, over the telephone or via video. We also run group sessions.
Important points to know:
- Our service is not a counselling/therapy service.
- Our service is not for crisis management or crisis intervention.
- We are unable to see patients who are under 18 years of age / patients with on going alcohol or drug dependency / perinatal cases / patients with complex mental health needs / patients who are already open to secondary community mental health team
- All referrals will be triaged by the team and patients who are deemed not suitable for our service will be highlighted to the GP surgery and a treatment recommendation will be proposed.
