What is the PCN Care Coordinator Service:
The NHS is a complicated system, it is not always easy to understand and navigate. Care Coordinators are here to support you through any confusion and hurdles you may encounter when using the NHS. The Care Coordinator team has knowledge and experience to help you find your way through the maze that is the NHS.
Examples of what the team can help with:
- NHS related digital support, e.g. help setting up the NHS app.
- Support patients to do self referrals to services.
- Support patients to navigate and engage with NHS services.
- Support patients to understand their clinical journey.

- Self referral: you can come to the front desk of our Waldron Health Centre office to inform us of what support you need, or you can call us on the phone, the details are below:
