Who can take part in this service?
- Having persistent pain for more than 6 months
- Not awaiting any investigations or procedures for pain
- Not part of any other chronic pain program
- Persistent pain post Covid
- Understanding and willing to adhere to the program
- Patients registered with a North LEWISHAM PCN GP practice
How do you use this service?
- You can refer yourself through your GP surgery reception
- Referred by GP
- Referred by other health care professional
How is this service delivered?
This service includes both Telephone and video call consultations in order to further reach people who may need this service
