Dr Edd Morris has been working with the Lewisham Health and Care Partners data team to develop a tool that can help GP’s identify patients at risk of health inequalities. This is done by using data already available within a patient’s records on exclusion, deprivation and multi-morbidity.
Once identified this can help in a number of ways from ensuring protected access for these patients and ensuring other support like the use of translators or other support services. For GP’s it identifies this person may need more support with their health from longer appointment times and offering a new patient review appointment to pick up any health problems early.
